Job summary

Ignite Rehab, operating within Metro Physical & Aquatic Therapy, is seeking a motivated Per Diem Occupational Therapist to provide in-home care in Morris County, NJ. The ideal candidate will deliver 1:1 treatment, evaluate patients, and develop individualized plans based on physician orders.

Must be a Graduate of an OT program and licensed in New Jersey or eligible. Travel to patients' homes within the assigned geographic territory is required; mentorship is available for documentation and

Qualifications

• Must be a graduate of an OT program.
• Licensed OT in New Jersey or eligible to obtain licensure.
• Home health experience preferred but not required; mentorship provided.
• Ability to travel to patients' homes within the assigned geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care.
• Educate family members, home health aides, and other caregivers on proper therapy techniques.
• Plan and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Must be willing to treat geriatric patients.
