Metro Physical & Aquatic Therapy is seeking a motivated Per Diem Occupational Therapist to provide in-home care in the New York area. This part-time role offers flexible scheduling and the chance to work with patients in their homes, delivering individualized therapy to improve function and independence.

Qualifications

• Licensed Occupational Therapist in NY or eligible.
• Graduate of OT program.
• Home health experience preferred; mentorship provided.
• Ability to travel within geographic territory.
• Strong communication with patients and team.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care.
• Educate family members, home health aides, and other caregivers on proper therapy techniques.
• Plan and carry out occupational therapy programs to maintain, improve or restore physical function and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Must be willing to treat geriatric patients.
