Metro Physical & Aquatic Therapy is seeking a motivated per diem Occupational Therapist for in-home care on the North Shore of Nassau County, NY. Open to all experience levels and willing to train new graduates.

Flexible schedule with 1099 and W-2 options, and a supportive team environment. The role involves 1:1 treatment, travel to patient homes, and determining individualized plans based on physician orders.

Qualifications

• OT program graduate required.
• Licensed OT in NY or eligible.
• Home health experience preferred; mentor provided for documentation.
• Ability to travel to patients' homes within geographic territory.
• Strong communication with patients, staff and management.

Responsibilities

• Accept and treat patients within designated geographic area.
• Provide 1:1 treatment.
• Develop individualized treatment plans based on physician orders.
• Deliver care per professional standards and plan of care.
• Educate family and caregivers on therapy techniques.
• Plan and carry out OT programs to maintain/improve function.
• Perform initial exam and determine diagnosis before intervention.
• Provide timely documentation for each visit.
• Evaluate treatment effects and adjust as needed.
• Willingness to treat geriatric patients.
