Job summary

Metro Physical & Aquatic Therapy is seeking a motivated per diem Occupational Therapist to provide in-home care in the North Shore area of Nassau County, NY. This role offers flexible scheduling and a per-visit pay model, with both 1099 and W-2 options.

The ideal candidate is a NY-licensed Occupational Therapist or eligible, capable of delivering 1:1 treatment, developing individualized plans, and educating families and aides while traveling to patients’ homes.

Qualifications

• Licensed Occupational Therapist in NY or eligible.
• Home health experience is preferred but not required.
• Ability to travel to patients' homes within assigned geographic territory.
• Strong communication with patients, caregivers and management.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care that aligns with professional standards and the plan of care.
• Educate family members, home health aides, and other caregivers on therapy techniques.
• Plan and carry out occupational therapy programs to maintain or improve function.
• Perform initial exam and document findings before intervention.
• Provide timely documentation for each visit.
• Evaluate treatment effects and adjust as needed.
• Willingness to treat geriatric patients.
