Job summary

Metro Physical & Aquatic Therapy is seeking a motivated per diem Occupational Therapist to provide in-home care in the North Shore area of Nassau County. This part-time role offers flexible scheduling and both 1099 and W-2 options to fit your preferred arrangement.

You will travel to patients' homes, deliver one-to-one treatment, evaluate needs, and develop individualized plans of care with mentor support for new graduates.

Qualifications

• Licensed Occupational Therapist in NY or eligible.
• Home health experience preferred but not required.
• Ability to travel to patients' homes within geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care.
• Educate family members, home health aides, and other caregivers on proper therapy techniques.
• Plan and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Must be willing to treat geriatric patients.
