Job summary

Habitat Health seeks a qualified Occupational Therapist to evaluate and treat PACE participants across acute, restorative and maintenance levels. You will join an Interdisciplinary Team, develop individualized plans, and conduct home visits when needed to maximize safety and independence in ADLs and exercise programs.

Qualifications

• Masters or Doctorate in Occupational Therapy from Accredited Program meeting AOTA & ACOTE standards.
• National Certification by the American Occupational Therapy Association or eligible for National Registration Examination of the American Occupational Therapy Association.
• A minimum of one year experience working with the frail or elderly.
• Bilingual: Spanish/English strongly preferred; additional languages a strong plus.

Responsibilities

• Provide occupational therapy evaluation and treatment on an acute, restorative and maintenance level to program participants.
• Participate in Interdisciplinary Team (IDT) by contributing insights from assessments, care plan recommendations, and care coordination.
• Perform home visits as needed and document activities including participant/caregiver response in participant’s chart.
• Educate participants and family members in proper care and use of various supports and exercise programs and DME use.
• Document therapy services provided and perform department record-keeping within documentation requirements.
• Assist in the ordering, inventory, distribution, and maintenance of durable medical equipment (DME).
