Job summary

Habitat Health is seeking an Occupational Therapist to deliver evaluation and treatment across acute, restorative, and maintenance levels for PACE participants. You will develop individualized plans, conduct home visits, and educate participants and caregivers on ADLs, exercises, and DME usage.

The role involves collaboration with the Interdisciplinary Team to inform holistic care planning, maintain regulatory documentation, and contribute to program standardization and quality audits.

Qualifications

• Masters or Doctorate in Occupational Therapy (MOT or DOT) or equivalent from Accredited Program meeting AOTA & ACOTE standards
• National Certification by the American Occupational Therapy Association or eligible for National Registration Examination of the American Occupational Therapy Association
• A minimum of one year experience working with the frail or elderly

Responsibilities

• Provide occupational therapy evaluation and treatment on acute, restorative and maintenance level to program participants
• Participate in Interdisciplinary Team (IDT) by contributing insights from assessments, care plan recommendations, and care coordination
• Assess and document conditions, create a treatment plan and discuss implementation with participants and caregivers
• Conduct home visits as needed and document activities in the participant’s chart
• Educate participants and family members on care and use of supports and DME
• Maintain department record-keeping and documentation requirements
• Assist in ordering, inventory, distribution, and maintenance of durable medical equipment (DME)
• Maintain quality assurance, safety, environmental and infection control programs
• Perform data collection, prepare reports and maintain participant statistics
• Attend staff meetings and patient care conferences as scheduled
