Habitat Health is seeking an Occupational Therapist to deliver OT evaluation and treatment across acute, restorative, and maintenance levels of care for PACE participants. You will develop individualized plans, conduct home visits when needed, and educate participants and families on safety, ADLs, and DME use.

As an IDT member, you will contribute to care planning, maintain compliant documentation, and support DME inventory and quality audits.

Qualifications

• Master's or Doctorate in Occupational Therapy (MOT or DOT) from accredited program meeting AOTA/ACOTE standards.
• National Certification by the American Occupational Therapy Association or eligible for National Registration Examination.
• Minimum of one year experience working with frail or elderly.
• Aligns with Habitat's purpose and values.

Responsabilités

• Provide OT evaluation and treatment across acute, restorative, and maintenance levels of care for PACE participants.
• Develop individualized treatment plans and conduct home visits as needed.
• Instruct participants, caregivers, and staff on maximizing safety and functional independence in ADLs and DME use.
• Contribute to IDT care planning and documentation meeting regulatory requirements.
• Assist with DME inventory management and participate in quality audits.
