Job summary

Trinity Health PACE is seeking an Occupational Therapist to provide comprehensive evaluations, treatments, and interventions for participants in day center, nursing home, and home-based settings. You will develop individualized care plans, implement restorative therapies to maintain independence and safety, and collaborate with the Interdisciplinary Team to achieve participant-centered goals.

Travel within the service area is required.

Qualifications

• Candidate must have an OT degree and state licensure.
• Experience with frail or elderly populations preferred.
• Strong assessment, treatment planning, and documentation skills.
• Experience with interdisciplinary care teams and caregiver education.

Responsibilities

• Perform comprehensive OT evaluations and develop individualized care plans.
• Implement therapeutic interventions to promote independence and safety.
• Collaborate with the Interdisciplinary Team to meet participant goals.
• Supervise and evaluate COTAs and contribute to quality and operations.
