Job summary

BoldAge PACE seeks an Occupational Therapist to join our elder-care team and provide comprehensive OT services to participants at the PACE center, in clinics, and in home settings. You will assess functional abilities, plan treatments, and collaborate with caregivers to support independence.

In this role you will lead and mentor OT staff, ensure compliant documentation, and participate in care coordination across the interdisciplinary team to deliver high‑quality, person-centered care for older

Qualifications

• Advanced degree in Occupational Therapy and licensed to practice in Illinois.
• Two years of interdisciplinary team experience preferred.
• Experience with frail or elderly population is desirable and training provided.
• Ability to participate in home care and community-based settings.

Responsibilities

• Plan, organize, and provide OT services as part of a PACE IDT.
• Assess function, home environment, and rehabilitation needs.
• Develop and revise individualized treatment plans with progress reviews.
• Deliver OT interventions in PACE center, clinic, and patient homes.
• Educate participants and caregivers on equipment use and safety.
• Maintain accurate documentation following standards.
• Supervise OTAs and mentor staff in safe practices.
• Support quality improvement and compliance initiatives.
