Job summary

Habitat Health in California seeks an Occupational Therapist to provide OT evaluations and treatments across acute, restorative, and maintenance levels for PACE participants. You will develop individualized plans, perform home visits, and educate participants and caregivers to maximize safety and independence.

As part of the IDT, you will contribute to assessments and care coordination, document findings, and ensure regulatory compliance.

Qualifications

• Masters or Doctorate in Occupational Therapy (MOT or DOT) or equivalent from Accredited Program meeting AOTA & ACOTE standards
• National Certification by the American Occupational Therapy Association or eligible for National Registration Examination of the American Occupational Therapy Association
• A minimum of one year experience working with the frail or elderly

• 2+ years as a practicing OT (Nice to have)
• Bilingual: Spanish/English strongly preferred; additional languages a strong plus (Nice to have)
• PACE experience and/ or ADCH/CBAS experience (Nice to have)
• A state issued driver's license, personal transportation, and auto insurance as required by law (Nice to have)

Responsibilities

• Provides occupational therapy evaluation and treatment on an acute, restorative and maintenance level to program participants.
• Participates in Interdisciplinary Team (IDT) by contributing insights from assessments, care plan recommendations, and care coordination.
• Assesses condition, creates a treatment plan and discusses implementation of therapy with participants and caregivers.
• Makes home visits as needed and documents activities including participant/caregiver response in participant’s chart.
• Educates participants and family members in proper care and use of various supports and exercise programs in addition to the use of DME.
• Documents therapy services provided and performs department record-keeping within documentation requirements
• Attends all staff meetings and patient care conferences as scheduled.
