Job summary

InnovAge seeks an Occupational Therapist to provide comprehensive OT evaluations and treatment plans for program participants, integrating care into the Plan of Care with the interdisciplinary team.

You will assess conditions, develop and discuss treatment plans with participants and caregivers, and document progress in charts while contributing to department records and QA activities. Collaboration with nursing home/AL staff is required.

Qualifications

• Graduate of an Occupational Therapy curriculum accredited jointly by AOTA.
• Bachelors in OT, MOT, or OTD.
• National Certification by AOTA or eligible for National Registration Examination.
• Registered with the state in which practicing.
• Requires state issued driver’s license, personal transportation.
• Minimum of one year experience with the frail or elderly.

Responsibilities

• Provide OT evaluations and treatment plans for participants.
• Assess condition, create treatment plan and discuss implementation with participant or caregivers.
• Integrate OT plan in the Team Plan of Care (POC).
• Maintain documentation and department records per policy and regulations.
• Educate participants and family members on care, exercise programs and use of DME.
• Assist physicians/IDT in evaluating function and revising plans as needed.
• Participate in team meetings and clinic educational programs.
