Job summary

Qualifications

• Masters or Doctorate in Occupational Therapy (MOT or DOT) from ACOTE‑accredited program.
• National Certification by the American Occupational Therapy Association or eligible for NBCOT exam.
• Minimum of one year experience working with the frail or elderly.

Responsibilities

• Provide occupational therapy evaluation and treatment on acute, restorative and maintenance levels for program participants.
• Develop individualized treatment plans and conduct home visits as needed.
• Instruct participants, caregivers, and staff on maximizing safety and functional independence in ADLs, exercise programs, and DME use.
• Participate in Interdisciplinary Team (IDT) with care coordination and assessments.
• Document therapy services and maintain participant charts per regulatory requirements.
• Support DME inventory management and contribute to quality audits and program development.
