Zusammenfassung

UnitedHealth Group's Optum Home & Community Care unit seeks a Nurse Practitioner to perform in-home assessments, manage acute issues, and coordinate care with an interdisciplinary team. This role requires daily travel to patient homes, with no night, weekend, or holiday shifts.

Eligible NPs must have ANCC/AANP certification, active licensure, DEA, and home-health experience. Competitive compensation includes a base salary, sign-on options, and comprehensive benefits.

Qualifikationen

Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
Active, unrestricted licensure in the practicing state
Active and unrestricted driver’s license with reliable transportation
Current, active DEA licensure/prescriptive authority or ability to obtain post-hire
Ability to gain a collaborative practice agreement where required
Ability to navigate varied environments and perform duties (sit/stand/kneel)

Aufgaben

Conduct comprehensive in-home assessments including history, meds review, vitals and physical exam
Manage members through acute or chronic conditions with testing and interventions
Support sub-acute rehabilitation episodes
Ensure accurate documentation including ICD-10
Collaborate with interdisciplinary care team
Conduct advanced illness and end-of-life conversations
Provide patient and caregiver education and counseling
