Job summary

UnitedHealth Group is seeking Nurse Practitioners or Physician Assistants for in-home care with no nights, weekends, or holidays. Daily travel to patient homes within assigned areas is required.

A strong focus on comprehensive home assessments, acute care, and rehabilitation coordination is emphasized. Compensation guidance indicates substantial earning potential with a $40,000 student loan repayment option or a $20,000 sign-on bonus for external candidates; travel logistics are a key component

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Current NCCPA certification for PAs and state licensure.
• Active, unrestricted licensure in the practicing state.
• Active and unrestricted driver's license with reliable transportation.
• Current DEA licensure/prescriptive authority or ability to obtain post-hire.

Responsibilities

• Conduct comprehensive in-home assessments including medical history review, vitals, and physical exams.
• Manage patients through acute events or chronic condition exacerbations; order tests and interventions as needed.
• Support short-term sub-acute rehabilitation episodes and ensure complete documentation (ICD-10).
• Collaborate with interdisciplinary care team and provide patient/caregiver counseling and education.
• Engage in advanced illness planning conversations and continuity of care across home settings.
