Job summary

Optum Home & Community Care is seeking a Nurse Practitioner to perform in-home assessments and manage care for patients across a defined geographic area. The role involves nightly/shift-less scheduling constraints with daily travel to patient homes, collaborating with an interdisciplinary team to plan and deliver care at home.

You will conduct comprehensive assessments, manage acute and chronic conditions, and document with ICD-10, while engaging patients and caregivers in education and care

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care

Responsibilities

• Conduct in-home assessments including past medical history review, medication reconciliation, vital signs, and physical exam
• Manage members through acute medical situations or chronic condition exacerbations
• Manage members during sub-acute rehabilitation episodes
• Ensure accurate documentation with ICD-10 coding
• Communicate and collaborate with the interdisciplinary care team
• Conduct advanced illness and care planning conversations
• Provide patients and caregivers with counseling and education
