Qualifications

• NP must be certified as Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted state licensure (or ability to obtain by start date).
• DEA licensure/prescriptive authority or ability to obtain post-hire.

Responsibilities

• Conduct in-home assessments and vitals, medication reconciliation and exam.
• Manage acute medical events and chronic condition exacerbations.
• Collaborate with interdisciplinary care team and document thoroughly.
