Job summary

Optum Home & Community Care in North Carolina seeks a Nurse Practitioner to join an interdisciplinary team delivering in-home clinical care. Travel within the assigned area is required, with no nights, weekends, or holidays in this position.

You will assess patients at home, manage episodes of acute and sub-acute care, document thoroughly, and collaborate with the care team to improve access to care. Requires active NP certification (ANCC or AANP), state licensure, DEA licensure/authority, and

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in the practicing state.
• Active and unrestricted driver's license with reliable transportation.
• Current, active DEA licensure/prescriptive authority or ability to obtain post-hire.
• Ability to gain a collaborative practice agreement, if applicable in your state.

Responsibilities

• Conduct comprehensive in-home assessments including medical history, medication reconciliation, vital signs and exams.
• Manage members through acute medical situations or chronic condition exacerbations.
• Oversee short-term sub-acute rehabilitation episodes.
• Ensure accurate documentation including ICD-10 codes.
• Collaborate with interdisciplinary care team and provide education to patients and caregivers.
• Engage in advanced illness and end-of-life discussions.
