Qualifications

• NPs must hold ANCC or AANP Family/Adult/Geriatric/AC Primary Care certification.
• Active, unrestricted licensure in state of practice.
• Active and unrestricted driver's license with reliable transportation.
• Current DEA licensure/prescriptive authority or ability to obtain post-hire.
• Ability to gain collaborative practice agreement per state.
• Ability to navigate varied environments.

Responsibilities

• Conduct comprehensive in-home assessments including medical history and vital signs.
• Manage acute medical or chronic condition episodes with testing and interventions.
• Coordinate care with the interdisciplinary team.
• Document thoroughly including ICD-10 codes.
• Discuss advanced illness and end-of-life planning with patients.
• Provide counseling and education to patients and caregivers.
