Job summary

Optum Home & Community Care is seeking a Nurse Practitioner to perform in-home assessments and provide primary care support for high-risk patients in their homes. This role involves daily travel within the assigned area, managing acute and chronic conditions, and collaborating with an interdisciplinary team to coordinate care.

Active ANCC/AANP family, adult or geriatric certification and an unrestricted license are required, plus a valid driver's license and DEA prescriptive authority or ability

Qualifications

• ANCC or AANP certification in family, adult, geriatric, or primary care required.
• Active and unrestricted state license without disciplinary actions.
• Active driver’s license with reliable transportation for daily home visits.
• DEA licensure or ability to obtain per state regulations.
• Ability to collaborate with a multidisciplinary team.
• Willingness to navigate varied home environments.

Responsibilities

• Perform comprehensive in-home assessments with history, meds, vitals and exam.
• Provide care planning, counseling and education to patients and families.
• Manage acute issues and chronic condition exacerbations in home setting.
• Document thoroughly including ICD-10 coding and care notes.
• Coordinate with interdisciplinary care team and caregivers.
• Discuss advanced care planning and patient goals with families.
• Ensure timely follow-up and appropriate referrals.
