Job summary

Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to deliver In‑Home Health Evaluations in members' homes. The role involves a 45‑minute visit, documenting medical history, performing a physical evaluation, and coordinating with health plans for additional tests when ordered.

Applicants should hold an active license in the coverage area, board certification, and be prepared for local travel across licensed states.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
