Job summary

Signify Health, part of CVS Health, is seeking a Part Time Clinician (NP or PA) to provide In-Home Health Evaluations, engaging with people where they live and helping bridge gaps in care.

Visits last about 45 minutes; you will review medications and history, perform a physical evaluation, and connect members to the right care. Expect local travel to licensed states and a flexible schedule.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).
