Job summary

Signify Health, part of CVS Health, is seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations at patients' homes, delivering patient-centered care and bridging gaps in health services.

Visits last about 45 minutes; you will review medications and history, perform a physical evaluation, and conduct optional tests when required by the plan. Travel locally and virtual visits are expected as needed.

Qualifications

• Active, unrestricted license in the coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Provide in-home health evaluations during 45-minute visits.
