Job summary

Signify Health, part of CVS Health, seeks Part Time Clinician (NP or PA) to perform in-home health evaluations. Visits are ~45 minutes, include med history review and physical assessment, with optional diagnostics as ordered by the member’s health plan.

You will use an iPad and flexible to travel within licensed states, including virtual visits. This part-time role offers benefits and requires active NP/PA licensure, board certification, and multi-state eligibility.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.
