Job summary

Signify Health, part of CVS Health, seeks a Part Time Clinician (NP or PA) to perform in-home health evaluations for members in their homes, bridging gaps in care and supporting a more personalized health experience.

The clinician will use an iPad-based workflow, conduct reviews of medications and history, perform physical evaluations, and may arrange additional tests as required by the health plan. Travel locally within licensed states is expected.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
