Job summary

Signify Health, part of CVS Health, seeks a Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations for members in their homes.

The role involves 29.5 hours per week, local travel to licensed states, and using an iPad with an intuitive workflow. You will assess health, review medications and history, and help connect members with timely care.

Qualifications

• Active, unrestricted license in coverage areas.
• Board certification required.
• Multi-state licenses preferred.
• Able to cover Greely MSA, Weld County.

Responsibilities

• Provide in-home health evaluations for health plan members.
• Travel locally to licensed states as needed.
• Use iPad and clinical workflow; no medicines prescribed or labs ordered.
• Coordinate with Signify Health services to close care gaps.

Licensed in CO and Weld County (coverage areas)
