Job summary

Signify Health, part of CVS Health, seeks a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations for plan members, delivering personalized assessments in the comfort of patients' homes.

Visits are about 45 minutes, including medication/history review and a physical exam; additional tests may be ordered by the health plan. Local travel within licensed states is required; virtual visits may occur as needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Provide in-home health evaluations for health plan members.
• Bridge gaps in care by engaging patients and caregivers.
• Review medications and medical history, perform physical evaluations.
• Travel locally within licensed states; conduct virtual visits when needed.
• Deliver Signify Health services including diagnostic and preventive tests.
