Job summary

Signify Health, part of CVS Health, is seeking a Part Time Clinician (NP or PA) to provide In-Home Health Evaluations across home visits. You’ll assess health needs in patients’ homes, perform physical evaluations, and connect members to the right care, while traveling locally and within licensed states.

You’ll travel locally and within licensed states, conduct visits using a streamlined workflow, and deliver Diagnostic & Preventive Services as ordered.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
