Job summary

Matrix Medical Network seeks an experienced NP to perform comprehensive in-home health assessments for senior and at-risk members. You will review medical history, medications, and social factors to identify gaps in care and support care planning with PCPs.

You’ll travel to patients' homes or SNFs, conducting thorough clinical evaluations and patient education. The role offers a competitive base salary with incentive programs and a 7,500 sign-on bonus.

Qualifications

• Master’s Degree in Nursing or commensurate experience.
• Current NP licensure in state of practice to include prescription authority or ability to obtain.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers; follow up as necessary.
• Coordinate patient education and follow up with PCPs.
• Provide services at the member’s residence, which may be in homes or Skilled Nursing Facilities.
