Job summary

Matrix Medical Network is seeking experienced NP/PA providers to conduct comprehensive in-home health assessments. Each visit lasts up to an hour and covers medical history, medications, and social determinants to close care gaps.

Providers will perform tests as needed, collaborate with care teams, and deliver services at patients’ residences or Skilled Nursing Facilities. You’ll travel up to 80-90% across the assigned state and neighboring areas.

Qualifications

• Master’s degree or commensurate NP/PA experience with licensure.
• Current NP/PA licensure in state of practice with prescription authority.
• Board certification in relevant NP/PA specialty (NCCPA/AANP/AACN/ANCC).
• Current BLS/ACLS/CPR certification.
• 1 year NP/PA experience.
• Ability to travel 80–90% of the time, including overnights.
• Valid state driver’s license and auto insurance.
• Strong computer skills and familiarity with medical/employee health software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, follow-up and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members and other staff as appropriate.
• Coordinate with PCP on patient education and follow-up.
• Provide services at members’ homes or SNFs.
