Job summary

Matrix Medical Network seeks experienced Nurse Practitioners to perform comprehensive in-home health assessments across assigned states. Each visit reviews medical history, current health, medications, and social factors to close care gaps and improve outcomes.

Providers travel up to 80-90% of the time, work flexible hours including weekends, and partner with PCPs, case managers, and families to deliver coordinated care. Competitive pay, mileage reimbursement, and retirement benefits are offered.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• AANP, AACN, or ANCC board certification in Adult/Family/Acute care specialties.
• BLS, ACLS or CPR certification.
• 1 year NP experience.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnoses, treatment, and referrals.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, social workers, and families.
• Provide patient education and follow-up per Matrix guidelines.
• Deliver services at members' residences or skilled facilities.

Education
