Matrix Medical Network offers in-home health assessments across the nation, delivering personalized care that improves health outcomes and reduces costs. As a Nurse Practitioner with national scope, you’ll work flexible hours, conduct geriatric assessments, and collaborate with a multidisciplinary team to close care gaps.

We support ongoing education, licensure expansion, and potential transitions to full-time roles, with mileage reimbursement and a strong benefits package.

Competenze

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure with prescriptive authority.
• Board certification in appropriate specialty (Adult/ Gerontology/ Family).
• Current BLS/ACLS CPR certification.
• 1 year NP experience preferred (new grads welcome).
• Valid driver’s license and auto insurance.

Mansioni

• Conduct adult/geriatric assessments including history, diagnosis, and education.
• Order and administer point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Educate patients and perform appropriate follow-up with PCPs and teams.
• Provide services at the member’s home or in Skilled Nursing Facilities.
