Job summary

Matrix Medical Network invites experienced Nurse Practitioners to join our in-home health assessment team. Our providers review health history, medications, and risks during visits that can last up to an hour, delivering care in patients' homes or skilled facilities nationwide.

Applicants must hold an NP license in their state, have board certification in Adult or Family specialties, CPR/BLS, and prefer at least one year of NP experience.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice to include prescription authority or ability to obtain prescriptive authority as needed.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care.
• Current BLS, ACLS or CPR certification.
• 1 year of NP experience preferred, new grads encouraged to apply!
• Valid state driver’s license for the state of residence.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation according to Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and family members as appropriate.
• Coordinate with PCP on patient education and follow-up as necessary.
• Provide services at the member’s residence, which can be in homes or Skilled Nursing Facilities.
