Matrix Medical Network is seeking a compassionate Nurse Practitioner to conduct comprehensive in-home health assessments for at-risk individuals, reviewing medical history and medication adherence to close health gaps. Visits may occur at the member's home or skilled nursing facilities, with documentation per Matrix protocols.

Formación

• NP licensure in state of practice with prescriptive authority.
• Board certification in NP specialties (adult/gerontology, family, or acute care).
• 1 year NP experience preferred; new grads encouraged.
• Valid driver's license and auto insurance as required.

Responsabilidades

• Perform adult/geriatric health assessments including medical history and documentation.
• Administer point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and family.
• Provide services at the member's residence, including homes or SNFs.
• Coordinate education and follow-up with PCPs.

Conocimientos

Excellent verbal and written comm.

Strong computer skills

Valid state driver's license

Educación

Master’s Degree in Nursing

NP licensure in state of practice

Board certification (AANP/AACN/ANCC)
