Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform Comprehensive Health Assessments in members’ homes or Skilled Nursing Facilities. Each visit lasts up to an hour to capture medical history, medications, social factors and health risks.

The role involves adult/geriatric assessments, ordering tests, collaborating with PCPs and care teams, and educating patients. FT opportunities require travel up to 80–90%, with benefits like mileage reimbursement, 401(k), and ongoing education.

Qualifications

• Master’s Degree in Nursing (NP) or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in appropriate NP specialty (adult, gerontology, family, etc).
• Current BLS/ACLS or CPR certification.
• 1 year of NP experience.
• Willing to travel up to 80–90% for extended periods including nights/weekends.
• Valid state driver’s license; auto insurance as required.
• Strong computer skills and familiarity with health-record software.

Responsibilities

• Conduct adult/geriatric assessments including medical history and diagnoses.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and families.
• Provide in-home/ facility-based health assessments and education.
• Document findings clearly and according to Matrix guidelines.
