Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments, reviewing medical history and current health to improve outcomes. You will collaborate with PCPs, case managers, and family caregivers, and may provide education and follow-up based on Matrix guidelines.

Responsibilities include conducting geriatric assessments, ordering tests when needed, and documenting clearly.

Qualifications

• NP licensure in state of practice and board certification in relevant specialty.
• Master’s degree in nursing or equivalent; active clinical credentials.
• BLS/ACLS or CPR certification required.
• Strong communication and computer skills with EMR proficiency.

Responsibilities

• Perform adult/geriatric health assessments in home or SNF settings.
• Document findings and coordinate referrals with PCP and care team.
• Administer point-of-care and lab testing as needed.
• Collaborate with physicians, case managers and family caregivers.
• Provide education and follow-up based on Matrix guidelines.

Education

Master’s Degree in Nursing / NP licensure

NP licensure in state of practice

Board certification in Adult/Family Gerontology/ACNP

BLS/ACLS or CPR certification
