Job summary

Matrix Medical Network provides in-home health assessments across the nation, reviewing medical history, medications, and social factors to close care gaps. Visits can last up to one hour and occur at the member’s home or in Skilled Nursing Facilities.

Qualifications

• Master’s degree or commensurate experience.
• Current NP licensure in state of practice.
• Board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care.
• Current BLS, ACLS or CPR certification.
• 1 year of NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license and auto insurance coverage.
• Strong computer skills and familiarity with health records software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, follow-up, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and family members to coordinate care.
• Coordinate with PCP on patient education and follow-up as necessary.
• Provide services at the member’s residence, including Homes or Skilled Nursing Facilities.
