Job summary

Matrix Medical Network seeks a Board-certified NP to perform comprehensive in-home health assessments across designated states. You will review health history, medications and social factors, providing follow-up and documentation per Matrix guidelines.

The role requires travel up to 80–90% with overnight stays; current NP licensure and prescriptive authority where needed are required; strong communication skills are essential.

Qualifications

• NP licensure in state of practice with prescriptive authority if needed.
• AANP/AACN/ANCC board certification in applicable NP specialty.
• 1 year NP experience.
• Travel up to 80-90% including overnight stays across states.

Responsibilities

• Conduct adult/geriatric assessments including medical history and health education.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCP, case managers, social workers and family/caregivers.
