Job summary

Matrix Medical Network delivers in-home clinical assessments across the nation, empowering providers to improve outcomes and reduce costs. Each visit includes reviewing health history, medications, and social factors to identify care gaps and tailor patient education.

The role emphasizes collaboration with physicians and care teams, offering flexible scheduling, travel opportunities, and ongoing education support. A valid NP license and state prescriptive authority where needed are required.

Qualifications

• Master’s Degree or commensurate experience and satisfactory NP licensure.
• Current NP licensure in state of practice with prescriptive authority if needed.
• AANP/AACN/ANCC board certification in Adult, Gerontology or Family specialties.
• BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members and caregivers.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence (homes or Skilled Nursing Facilities).
