Job summary

Optum Home & Community Care is expanding its HouseCalls program, seeking a Nurse Practitioner to perform in-home health assessments for Medicare Advantage and other plans in a field-based role. This non-prescribing position focuses on education, gap closure, and collaboration with primary care providers.

You will drive care coordination, address social determinants of health, and operate with a flexible schedule during 8am–8pm across a 7-day week, with no on-call, weekends, or holidays.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in the state of Texas (or able to obtain by start date) without disciplinary actions or practice limitations.
• Active and unrestricted driver's license.
• Access to reliable transportation for home visits.
• Ability to navigate varied home environments; ability to sit/stand/kneel as needed to perform assessments.
• If NP/PA independence allowed, obtain licensing board approval within required timelines.

Responsibilities

• Conduct comprehensive in-home assessments including medical history, medications, vitals, and exam.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to patients' PCPs to address care gaps.
• Recognize urgent situations and intervene appropriately.
• Educate patients on diseases, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
