Job summary

Optum Home & Community Care, part of Optum, is expanding its HouseCalls program. As an Advanced Practice Clinician (Nurse Practitioner), you will perform in-home health assessments for Medicare Advantage and other plans, focusing on education, gap closure, and collaboration with PCPs to improve health outcomes.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Unrestricted licensure in the practicing state without disciplinary actions.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other plans.
• Document diagnoses for care management and plan treatment.
• Communicate findings to members' PCPs to close care gaps.
• Educate members on disease processes, medications, and adherence.
