Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive in-home health assessments. You will review health history, medication adherence, and social factors to identify care gaps and improve outcomes.

Responsibilities include conducting geriatric assessments, ordering tests, coordinating with physicians, and delivering care at patients’ residences or SNFs. Expect substantial travel (80–90%) and opportunities for FT roles with a sign-on bonus.

Qualifications

Master’s degree or commensurate experience and NP licensure.
Active NP licensure in state of practice with prescriptive authority.
Board certification in Adult/Adult Gerontology/Family/ Emergency/ Acute Care.
Current BLS/ACLS/CPR certification required.
Minimum 1 year of NP experience.
Ability to travel up to 80-90% including overnight when needed.
Valid state driver’s license and auto insurance.
Proficient computer skills and experience with health record software.

Responsibilities

Conduct adult/geriatric health assessments including medical history, diagnosis, treatment, education and referrals.
Administer and order point-of-care and lab testing as needed.
Collaborate with physicians, case managers, social workers, and families.
Coordinate with PCPs for patient education and follow-up.
Provide services at patients’ homes or in Skilled Nursing Facilities.
