Job summary

Matrix Medical Network seeks experienced Nurse Practitioners to perform comprehensive in-home health assessments, reviewing medical history, medications, and social factors to identify care gaps. You will coordinate with physicians and care teams to ensure appropriate follow-up and education for members in their homes or SNFs.

Responsibilities include conducting assessments, ordering tests, and documenting results per Matrix protocols; opportunities for FT roles and state licensure support are

Qualifications

• Master’s degree in nursing with NP licensure.
• Active NP licensure in state of practice.
• Board certification in one of applicable NP specialties.
• Current BLS/ACLS or CPR certification.
• Strong communication and documentation skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history and treatment plans.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, social workers, and family members.
• Provide in-home health assessments at member residences or SNFs.
• Document findings clearly following Matrix guidelines.
