Job summary

Matrix Medical Network is seeking a Nurse Practitioner to conduct comprehensive in-home health assessments for members, lasting up to one hour. You will review medical histories, medications, and social determinants, and coordinate care with physicians and care teams to close care gaps.

Responsibilities include ordering tests, documenting findings per Matrix protocols, and delivering education and follow-up in members’ homes or SNFs.

Qualifications

• Must hold current NP licensure in state of practice with prescriptive authority or eligibility.
• Current NP licensure and board certification in Adult, Family or Gerontology specialties are required.
• BLS/ACLS or CPR certification
• Minimum 1 year NP experience preferred; new grads encouraged to apply
• Valid driver's license; auto insurance coverage required in resident state
• Strong computer skills and familiarity with health-record software
• Excellent verbal and written communication skills

Responsibilities

• Perform in-home comprehensive health assessments lasting up to one hour.
• Review medical history, diagnoses, treatment plans, and medication adherence.
• Order and administer point-of-care and lab testing as needed.
• Coordinate with PCPs, case managers, social workers, and other care teammates.
• Provide care at members' residences, including homes or skilled nursing facilities.
