Matrix Medical Network seeks an experienced Nurse Practitioner to perform in-home comprehensive health assessments, reviewing medical history, medications, and social determinants at patients' homes or SNFs.

You will collaborate with physicians and care teams to close care gaps, provide education, and order tests as needed. A strong credentialing slate and travel availability are required.

Formación

• Master's degree and NP licensure in the practicing state.
• Certification in relevant NP specialty (adult/gerontology/family).
• Current BLS/ACLS or CPR certification.

Responsabilidades

• Conduct adult/geriatric assessments including medical history and health education.
• Administer point-of-care and lab tests; document per Matrix guidelines.
• Collaborate with PCPs, case managers, social workers, and caregivers.
• Provide services at members' residences, including homes or SNFs.

Conocimientos

NP licensure in state of practice
