Job summary

Optum HouseCalls, a part of UnitedHealth Group, seeks an Advanced Practice Clinician (NP/PA) to perform in-home Medicare assessments across Rhode Island. Flexible schedules range from 20 to 40 hours per week, with no on-call requirements and no weekends or holidays.

Spanish-speaking ability is required to serve diverse patient populations at home visits. This field-based role focuses on education, gap closure and collaboration with PCPs to improve health outcomes, while providing a balanced work

Qualifications

• Active certification (NP or PA) and state licensure.
• Unrestricted driver’s license.
• Access to reliable transportation for home visits.
• Ability to navigate varied home environments.

Responsibilities

• Perform in-home health assessments including medical history, vitals, examinations.
• Conduct screenings and point‑of‑care testing as appropriate.
• Document diagnoses for care management and treatment planning.
• Communicate findings to primary care providers to close care gaps.
• Identify urgent issues and intervene when needed.
• Educate patients on diseases, medications and adherence.
