Job summary

OptumCare is seeking a Nurse Practitioner or Physician Assistant to perform comprehensive in-home health assessments for Medicare Advantage and other plans across Connecticut and Rhode Island. This field-based role emphasizes preventive care, patient education, and collaboration with primary care providers to close care gaps.

Ideal candidates are Spanish bilingual with 1+ years of clinical experience, hold active NP or PA certifications, and have valid licensure.

Qualifications

• Active certification (ANCC or AANP) for NP; Family, Adult, Gerontology Primary Care.
• Current NCCPA certification and state licensure for PAs.
• Active, unrestricted licensure in practicing state.
• Active driver's license and reliable transportation for home visits.
• Spanish bilingual ability.
• 1+ years of clinical experience (family, geriatric, or home health preferred).

Responsibilities

• Conduct comprehensive in-home health assessments.
• Partner with PCPs to address care gaps and improve outcomes.
• Educate patients and help connect them with community resources.
• Travel to patients across CT and RI (dual-state) with travel expenses covered.
