Job summary

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in the practicing state.
• Active and unrestricted driver's license.
• Access to reliable transportation to complete home visits.
• Bilingual in Spanish.

Responsibilities

• Conduct comprehensive in-home health assessments.
• Help identify and close care gaps for patients.
• Educate patients on disease management, medications, and preventive care.
• Address social determinants of health and connect patients with community resources.
• Operate as part of an interdisciplinary care team to support patient outcomes.
