Job summary

UnitedHealth Group’s Optum HouseCalls team conducts in-home health assessments for Medicare Advantage members. The role emphasizes flexible schedules (20–40 hours/week), no on-call, weekends, or holidays, with a focus on education, gap closure, and collaboration with PCPs.

Qualifications

• Spanish-speaking required.
• Experience with electronic medical records preferred.
• Active licensure in practicing state.

Responsibilities

• Perform in-home health assessments for Medicare Advantage members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to primary care providers to close care gaps.
• Educate patients on disease processes, medications and adherence.
• Address social determinants of health and arrange referrals.
