Job summary

UnitedHealth Group's Optum Home & Community Care is seeking an Advanced Practice Clinician (NP or PA) to conduct in-home Medicare assessments. This field-based role focuses on education, gap closure, and collaboration with PCPs to improve outcomes, with no on-call, weekends, or holidays and flexible scheduling between 8am–8pm.

This role is part of the HouseCalls team delivering preventive care in patients' homes, focusing on improving health outcomes through education and care coordination.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• For PAs: NCCPA certification and state licensure.
• Active, unrestricted licensure in the practicing state with no disciplinary actions or practice limitations.
• Driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.

Responsibilities

• Conduct in-home assessments including history, medications, vitals, and exam.
• Perform screenings and point-of-care tests as appropriate.
• Document diagnoses for care management and communicate findings to PCPs.
• Identify urgent situations and intervene as needed.
• Educate patients on disease processes and medications.
• Address social determinants of health and provide referrals.
